problem that lasted for about 6 months ?

ok so i have been having a lot of problems with my rectum for some reason. i have been having different types of pain for the last 6 months it first started with a hemorrhoid i went to the doctor and she gave a cream it kinda fixed the problem but after like a week or maybe even less i had different kinds of pain i went to the doctor again and she said it is a fissure this time and i had to go to a surgeon and he gave me another cream to use but this time it didnt work. im in SO MUCH pain every time i have a BM while im going and after im done. the pain lastast for at least 12-24 hours and it is really bad nothing can make it go away and im not sure what to do. please if you have any suggestions or ideas about what this is or what can i do to make it go away you can write an answer.
